Managed IT vs Co-Managed IT: Which Model Fits Your Business?

If your business is past the "everyone calls the boss's nephew when something breaks" stage, you've probably looked at hiring outside IT help. And you've probably noticed there are two distinct models pitched to mid-sized businesses: managed IT services and co-managed IT services.

They sound similar. They're priced similarly. They serve very different situations.

Managed IT: the outsourced IT department

Managed IT services means an outside firm runs your IT entirely. You don't have any internal IT staff — the managed services provider (MSP) is your help desk, your network engineer, your cybersecurity team, your Microsoft 365 administrator, your patch management, and your strategic IT advisor, all rolled into one monthly bill.

This is the right model when:

  • You have fewer than ~60 employees
  • You don't have anyone internal whose job description is "IT"
  • You want a single accountable partner for every IT issue
  • You don't want to hire, train, or manage internal IT staff

Co-managed IT: augmenting your existing team

Co-managed IT means you have internal IT staff AND an outside firm working alongside them. The boundaries get drawn deliberately: maybe your internal IT person handles the day-to-day help desk and on-site work, while the co-managed partner provides the security tools, the after-hours coverage, the specialized expertise, the documentation, and the strategic depth that one person can't realistically maintain alone.

This is the right model when:

  • You have one or two internal IT people who are stretched thin
  • Your internal IT staff is great at the day-to-day but doesn't have time for cybersecurity, compliance, or strategy
  • You want to keep your existing IT relationships intact but get more horsepower
  • You're growing fast and need IT to scale faster than your hiring pace allows

How the costs compare

For an apples-to-apples 50-person business:

  • Managed IT runs $130–$250 per computer per month + $220–$300 per server per month, all-in
  • Co-managed IT typically runs $60–$120 per computer per month (because your internal staff is doing a portion of the work) PLUS the salary of your internal IT staff

Total cost of co-managed often exceeds total cost of managed at the same headcount, because you're paying for both an internal team and an external partner. But you're getting more capacity, more redundancy, and more depth in specialty areas like cybersecurity.

The hybrid trap to avoid

The worst version of co-managed is when nobody actually owns anything. Your internal IT person assumes the MSP is handling backups; the MSP assumes the internal person is. Tickets sit because both sides expect the other to pick them up. Patches don't get applied because the boundary was never written down.

A good co-managed engagement has a written responsibility matrix that lists every IT function and assigns it to one of three columns: "internal owns," "MSP owns," or "joint." If your prospective co-managed partner can't produce that document, the relationship will fail by month four.

How to choose

Three questions that usually settle the call:

  1. Do you currently have any internal IT staff you intend to keep? If no → managed. If yes → co-managed.
  2. Is your internal IT person/team currently overwhelmed? If yes, co-managed is almost certainly the move. Hiring a second internal IT person costs ~$85,000–$130,000/year fully loaded. A co-managed engagement gets you more capacity for less.
  3. Are you under any compliance obligation (HIPAA, CMMC, PCI)? If yes and you have internal IT, strongly consider co-managed — the compliance documentation and tooling load is usually too much for a small internal IT function to handle alone.

Switching between the models

Most clients we work with end up evolving over time. A 20-person business hires us for managed IT. They grow to 50 people, hire their first internal IT person, and convert to co-managed. They grow to 120 people, hire a second internal person, and stay co-managed but redraw the boundary — we hand more day-to-day work back and focus on security, strategy, and the second-line escalation queue.

Either model is reversible. The relationship matters more than the label.
